Previous silicone oil exchange may affect retinal re-detachment after surgery

FORT LAUDERDALE, Fla. — Recurrent retinal detachment can occur after silicone oil removal following retinal detachment surgery. A study investigating risk factors found previous silicone oil exchange as the only significant association.

“Despite advances in surgical technique and equipment, the rate of retinal re-detachment following [silicone oil removal] is high. Few studies have examined potential risk factors, with conflicting results,” Matthew R. Starr, MD, said at the Retina World Congress.

The retrospective analysis included 205 eyes with silicone oil tamponade during rhegmatogenous retinal detachment repair with subsequent oil removal from Dec. 1, 2014, to March 1, 2020, at Wills Eye Hospital. The primary outcome was the rate of retinal re-detachment following silicone oil removal.

Proliferative vitreoretinopathy at the time of initial retinal detachment repair was documented in 143 eyes (69.8%). The macula was detached in 176 eyes (85.9%) at the time of silicone oil instillation, 150 eyes (73.2%) received prophylactic laser, and 92 eyes (44.9%) had a buckle at the time of silicone oil removal. Thirty-eight eyes (18.5%) had retinal re-detachment following silicone oil removal.

There were no identifiable preoperative or intraoperative characteristics associated with re-detachment rates.

“Many factors potentially associated with re-detachment were analyzed, including cataract or concomitant cataract surgery, preexisting buckle or retinectomy, laser at the time of [silicone oil removal] and several others, but the only significant factor was, not surprisingly, previous [silicone oil] exchange. Interestingly, longer [silicone oil] tamponade was associated with improved anatomic outcomes,” Starr said.
